What is the job role?This is an exciting opportunity to work with individuals experiencing homelessness, mental health crisis and are at risk of psychiatric inpatient admission. You will join a team based in a busy Home Treatment Team that is promoting the bio-psycho-social model of mental disorder, and your role will be intrinsic in promoting the social model of practice. Your work will be stimulating and challenging, and you will work very closely with NHS and Local Authority Homelessness Services, including commissioned and non homelessness accommodation providers as well as an experienced team of Social Workers, AMHPs, Psychiatrists, Nurses and Support Workers and other partner agencies. You will be expected to:- have a prominent role in supporting individuals requiring vital and urgent social support with a focus on preventing homelessness and or sustaining accommodation Complete personalised assessments of social needComplete assessments under Care Act (2014) and identify and arrange social interventions accordingly that specifically aim to achieve recovery and wellnesswork closely with a whole range of Statutory and Third Sector colleagues to assess, identify and arrange interventions based on social and recovery models. always offer a compassionate and respectful response that inspires hope and recovery This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
