Right now, we are looking for a Tenancy Sustainment Coordinator to play a pivotal role in our Complex Needs Service in Hammersmith and Fulham.Sounds great, what will I be doing?This role provides tailored support to former rough sleepers and those at risk, aiming to promote independence, wellbeing, and long-term tenancy sustainment. You will work collaboratively with service users to co-produce assessments and SMART support plans, addressing a wide range of needs such as mental health, substance misuse, and complex life experiences. Key-working sessions will be used to support personal development, access to healthcare and treatment services, engagement in meaningful activities, and improved financial stability through benefits support, training, and employment opportunities. The role involves close liaison with multi-agency partners, to ensure coordinated care. A co-production approach underpins the service, empowering users as partners and promoting mutuality over dependency. Flexibility is essential, with work required outside standard hours. W

The knowledge you should hold will be across housing and homelessness legislation, welfare and benefits legislation, and causes of homelessness. You will hold excellent customer service skills and have he ability to create interventions with services users to address the causes of homelessness.
